WHAT SHOULD I BRING TO MY FIRST APPOINTMENT?
Please bring the following items with you: Picture ID, List of Medications, Proof of Insurance, List of Doctors, Labs/Medical Records/Specialist Visits.
DO YOU OFFER FREE HEALTH SERVICES?
No. A sliding scale is offered for uninsured patients who cannot afford services.
WILL THE PRACTICE PROVIDE EMERGENCY SERVICES?
No. Emergency services, hospitalizations, major surgeries, or expensive diagnostic services will not be performed. If you have an emergency, you should always call 9-1-1 and have an ambulance, or someone else, drive you to the nearest emergency room!
WHAT SERVICES ARE PROVIDED?
Primary and gender affirmation care, HIV testing, Hep C testing, women's health (not Obstetrics), men's health, immunizations, and lab work.
WHAT HOURS WILL THE PRACTICE BE OPEN?
At the present time, our hours of operation are Monday through Friday, 9:00 am - 5:00 pm, and one Saturday per month by appointment only. We do not offer walk in services, but a same day appointment may be possible subject to schedule availability. Please call our office at (540) 9993-4473, for further information.
DO YOU ACCEPT INSURANCE?
We accept many insurance plans and are continually adding new plans. To see if we accept your health plan, please call us at (540) 993-4473.
WILL MY INSURANCE COMPANY COVER ALL VISIT COSTS?
To best understand the specific details of your policy and its coverage, please contact your insurance provider directly.
DOES MOSAIC CARE CENTER OFFER SERVICES IN SPANISH?
Yes, we have bilingual staff who are capable of translating for you if necessary.
WHAT IS THE CANCELLATION POLICY?
Please call us directly to reschedule or cancel an appointment. Patients who fail to show for their scheduled appointment, or did not notify the office within 24 hours of their scheduled appointment time, may be subject to a “No Show” fee.
WILL MY PROVIDER SEE ME AND/OR TREAT ME IN THE HOSPITAL?
No. You will be seen and cared for by the Specialist and hospital staff. Your Provider at Mosaic Care Center will have access to the labs and charts.
HOW ARE PRESCRIPTIONS AND REFILL REQUESTS HANDLED?
Initial medication prescriptions and refills are given at in-person appointments. However, if a follow-up appointment is missed and a patient needs a refill, this will be addressed on a case by case basis.
Please plan in advance for your refill requests. Do not wait until you are completely out of your medication to request a refill. Be sure to allow extra time for weekends and holidays. If you should run out of your medication on a weekend or holiday, there will be a delay in refilling your prescription until we re-open.
DO YOU TREAT ALL AGES?
No. At this time, we are accepting patients that are 18 years of age and older.
DOES THE MOSAIC CARE CENTER PROVIDE GENDER AFFIRMING SERVICES?
Yes. The mosaic Care Center is a designated safe space where patients will experience gender affirming medical care provided by culturally competent staff.
