WHAT SHOULD I BRING TO MY FIRST APPOINTMENT?
Please bring the following items with you: Picture ID, List of Medications, Proof of Insurance, List of Doctors, Labs/Medical Records/Specialist Visits.
DO YOU OFFER FREE HEALTH SERVICES?
Yes. We offer HIV counseling and testing, Hepatitis C testing, and Syphilis testing.
WILL THE PRACTICE PROVIDE EMERGENCY SERVICES?
No. Emergency services, hospitalizations, major surgeries, or expensive diagnostic services will not be performed. If you have an emergency, you should always call 9-1-1 and have an ambulance, or someone else, drive you to the nearest emergency room!
WHAT SERVICES ARE PROVIDED?
Primary and gender affirmation care, HIV testing, Hep C testing, women's health (not Obstetrics), men's health, immunizations, and lab work.
WHAT HOURS WILL THE PRACTICE BE OPEN?
At the present time, our hours of operation are Monday through Friday, 9:00 am - 5:00 pm, and one Saturday per month by appointment only. We do not offer walk in services, but same day appointments are often available. Please call our office at (540) 907-4555 for further information.
DO YOU ACCEPT INSURANCE?
At this time we only accept Medicaid and Medicare; however, we are continually working to accept new insurance providers.
WILL MY INSURANCE COMPANY COVER ALL VIST COSTS?
To best understand the specific details of your policy and its coverage, please contact your insurance provider directly.
DOES MOSAIC CARE CENTER OFFER SERVICES IN SPANISH?
Yes, we have bilingual staff who are capable of translating for you if necessary.
WHAT IS THE CANCELLATION POLICY?
Please call us directly to reschedule or cancel an appointment. You will be charged the full fee for any appointment cancelled less than 24 hours in advance.
WILL MY PROVIDER SEE ME AND/OR TREAT ME IN THE HOSPITAL?
No. You will be seen and cared for by the Specialist and hospital staff. Your Provider at Mosaic Care Center will have access to the labs and charts.
HOW ARE PRESCRIPTIONS AND REFILL REQUESTS HANDLED?
Initial medication prescriptions and refills are given at in-person appointments. However, if a follow-up appointment is missed and a patient needs a refill, this will be addressed on a case by case basis.
Please plan in advance for your refill requests. Do not wait until you are completely out of your medication to request a refill. Be sure to allow extra time for weekends and holidays. If you should run out of your medication on a weekend or holiday, there will be a delay in refilling your prescription until we re-open.

No. At this time, we are accepting patients that are 18 years of age and older.
